How to Submit Documentation For Accomodation. Â
Education Pathways is committed to providing opportunities for all qualified students to participate fully in the academic environment. Education Pathways does provide reasonable accommodations for individuals with disabilities.
Students must provide a minimum of 7-10 business days' notice in order for their accommodation to be properly processed.

Submit your request and documentation via this form to Education Pathways. Note: Missing or incomplete documentation may result in a delay of processing or decline of the request.
ONCE THE FORM HAS BEEN SUBMITTED:
Education Pathways receives the request and reviews the submitted form and documentation.
The Disability Coordinator will review and make the final decision.
Education Pathways will notify the student of the final decision.
